By Kelly, Seattle United StatesI found out that I was pregnant with monochorionic diamniotic twins at 8 weeks when I started to bleed. The ultrasound showed a moderate sized subchorionic hematoma. The hematoma resolved but my girls developed twin to twin transfusion syndrome (ttts) at 19 weeks. I had laser surgery to fix ttts at 19+1 and my waters broke with baby B shortly there after. The leak sealed somewhat but she had very low fluids (deepest pocket of ~2cm) until both of my babies waters broke at 25+4. I stayed on very strict bed rest from 19 weeks on and did not have any internal exams. My mfm team was very aggressive with my care and gave me magnesium, steroids, ibuprofen, nifedipine and monitored my babies multiple times per day. I was careful to lay in the position where their cords had good blood flow and only got up to use the bathroom and for a 5 min shower each day. Everything else I did while laying in the "safe position". I was very lucky and made it to an elective c-section at 34 weeks 0 days. Baby A came out first and was very healthy. Apgars of 9 and 9! Baby B had a very rough start because of the low fluids that she had had since 19 weeks. She needed to be intubated and developed a pneumomediastinum (air that escaped out of a hole her lung into the tissues around her heart). She has smaller lungs than her sister but has excellent oxygen saturations and isn't limited in any way (they are now 18 months old). We were very lucky in our outcome but I feel that bed rest and having a team of doctors that treated me with all of the available medications to hold off labor gave my girls a good chance.
PROM at 19 weeks + 1 days. Delivery at 34 weeks.
